PRIVATE GROUP AGREEMENT

PLEASE READ THE FOLLOWING INFORMATION AND SIGN BELOW:
The following information will help your group get the most out of private yoga session(s) and clarify the instructor/private group relationship. Yoga is more than physical exercise. It is a transformative practice that integrates body, mind and spirit to arrive at deeper levels of relaxation and awareness.

I understand that I am agreeing to the following terms, on behalf of MY GROUP:

• All exercise programs involve a risk of injury. By choosing to participate in a private group yoga class(es), each member of MY GROUP understands that he or she is solely responsible for his or her decision to practice yoga and he or she voluntarily assumes a certain risk of injury.
• The venue and instructors of these classes shall not be held liable for any injury, loss or damage to property and/or persons sustained during or as a result of participation in classes, workshops, retreats and other events held by Katy Dunlap Yoga. 
• By choosing to participate in an in-person class, MY GROUP agrees to adhere to policies and procedures established by Katy Dunlap Yoga in response to COVID-19. MY GROUP understands that such rules are designed to protect public health and safety and to comply with state and local regulations, and that such rules may be updated as new information becomes available. MY GROUP understands that hands-on assists may not be offered. Each member of MY GROUP agrees that if he or she is not feeling well, or is exhibiting  symptoms of COVID-19 or has been exposed to anyone diagnosed with COVID-19 in the past 5 days, he or she will not attend class. 
• ​By signing this agreement and unless expressly stated otherwise, MY GROUP permits the use of any photos that have been taken during in-person classes, workshops and retreats held by Katy Dunlap Yoga, for use by Katy Dunlap Yoga for promotional purposes.
