PLEASE READ THE FOLLOWING INFORMATION AND SIGN BELOW.
The following information will help you get the most out of your yoga classes and clarify our instructor/student relationship. Yoga is more than physical exercise. It is a transformative practice that integrates body, mind and spirit to arrive at deeper levels of relaxation and awareness. All exercise programs involve a risk of injury. By choosing to participate in yoga classes, you voluntarily assume a certain risk of injury.
Awareness is fundamental to the practice of yoga. By attending this class, I affirm that I am solely responsible for my health and well-being, as well as my decision to practice yoga, a program involving physical exercise. I understand that yoga is not recommended and is not safe under certain medical conditions. I do not have any physical or medical conditions that would limit my participation or preclude participation in an exercise program. I agree that the venue and instructors of these classes shall not be held liable for any injury, loss or damage to property and/or persons sustained during or as a result of participation in classes, workshops, retreats and other events held by Katy Dunlap Yoga. I agree to listen to my body and monitor myself during every session.
I understand that by choosing to participate in an in-person class, I am agreeing to adhere to the current Center for Disease Control (CDC) recommendations, New York State's guidance, and any additional policies adopted by Katy Dunlap Yoga in response to COVID-19. I understand that such rules are constantly changing as new information becomes available and I agree to review the current rules and regulations on a regular basis. I understand the following:
I understand that equipment will not be available for use in class and that I am responsible for providing and sanitizing my own equipment. I understand that hands-on assists will not be offered. By signing this agreement, I permit the use of any photos, where my likeness is represented, that have been taken during in-person classes, workshops and retreats held by Katy Dunlap Yoga, for use by Katy Dunlap Yoga for promotional purposes.

- Current guidance distinguishes between fully vaccinated and unvaccinated individuals.
- Fully vaccinated individuals may choose not to wear a face covering and are not required to physically-distance before, during or after class. A person is considered fully vaccinated after 2 weeks have passed since he/she/they received the second dose of either the Pfizer or the Moderna vaccine, or after 2 weeks have passed since he/she/they received the single dose of the Johnson & Johnson vaccine.
- All other individuals are considered unvaccinated. Unvaccinated individuals must still physically- distance (6ft) from others and wear acceptable face coverings when 6ft distance is not possible (i.e. in common areas and at other times when not on the mat).
